An exciting opportunity has arisen for an experienced Physiotherapist with diverse clinical skills to join our Community Rehabilitation Team based in the South Hams. The service provides a patient-focused multi-disciplinary approach to supporting discharge from hospital, long term conditions management, maximising independence and preventing admission through crisis intervention
