Our organisation is looking for a Care Manager to join our team. The successful candidate will be responsible for managing the care of our clients, providing support and guidance to their families, and ensuring that all services are provided in a timely and efficient manner. The Care Manager will also be responsible for developing and implementing individualised care plans, monitoring progress, and providing ongoing assessments of the client’s needs. The ideal candidate will have a passion for helping others and a commitment to providing quality care.

Responsibilities:

• Provide crisis intervention services to clients in need.
• Develop and implement individualized care plans for clients.
• Conduct home visits to assess client needs and provide support.
• Monitor client progress and adjust care plans accordingly.
• Provide referrals to appropriate community resources.
• Collaborate with other healthcare professionals to ensure quality of care.
• Maintain accurate records of client progress and services provided.
• Provide education and training to clients and their families on relevant topics.

Job Types: Full-time, Permanent

Salary: £31,110.00-£34,114.00 per year
